Event Request Form

Please fill out our events form below. We require event organizers to register their events with SickKids Foundation to ensure we provide the appropriate support and to ensure the community is made aware of the events that support SickKids.

Please note that when a business or individual wishes to donate a portion of the proceeds from the sale of a product or service this is a cause related marketing venture and will not be treated as a community event. Please contact the Foundation and ask for the Cause Related Marketing Coordinator at 416.813.6166.

SickKids Foundation is not able to accept funds raised through Texas Hold' Em Poker events as this activity cannot be licensed by the Ontario Alcohol and Gaming Commission. Other social gaming activities require significant lead time and fundraising revenue to warrant consideration by SickKids Foundation.
